My signature below confirms that my son has permission to participate in off campus activities and trips throughout the school year. I understand that this involves travel to and from the Lev Simcha campus. Activities can be identified as but is not limited to extracurricular activities, fun trips, field trips, and Shabbatons.
TRANSPORTATION WAIVER:
I also understand that private drivers, which may include my son (pending my signed permission below), or another student, may be used to transport students to and from the activity. The Yeshiva's insurance does not cover damages arising from, or related to, the operation of any private vehicle, failure to follow the directed driving route, or any personal negligence related to this activity.

I also understand that I have the ability to refuse to sign this Form. In addition, that if I refuse to sign, my son will be required to use parent provided transportation.
ACKNOWLEDGEMENT OF PERSONAL LIABILITY AND WAIVER:
By signing this form, I accept full responsibility and liability for the actions of my student as a driver and or passenger. I understand the risks associated with granting permission for my student to drive or ride with another student driver. I understand that my student must abide by all schools in addition to adhering to the law as they operate or ride in another student’s vehicle.
SIGNATURE:
I confirm that I have carefully read this consent and agree to its terms knowingly and voluntarily. I also confirm that I am the parent or legal guardian of the student. This consent has been read and is understood by me.
